I am happy to share that we have fixed this bug in our latest release – 23.0.6 which is available worldwide now.
Going forward, our goal is to fix as many top User-Voice bugs as possible and as frequently as possible. Given the nature of the fixes, some of the bugs will take a longer time to fix, but we are on it.

I ran into the same issue. (Caused me to misprint about 300 bucks worth of product.)
I used an overprint on the gradated stroke of an object ( Spot color gradation of 100% spot color and 0% of same spot color). Everything viewed correctly in Overprint Preview..then I saved it. When I opened the file back up it still looked correct in overprint preview even though the overprint stroke was now in-checked. Unfortunately just because Illustrator previewed it correctly the printer does not. I can't get it to save and stay an overprint... What gives Adobe. I really need this to work and it's not and there isn't anyway to be sure it's good before it goes to print. If we're talking about a large layout I could looks about 500 bucks in plastic everything Illustrator glitches out on this.
Please fix.
