Element appears in Illustrator

We received a customer’s file in PDF. We always work the file in Illustrator.
In this case, when we opened the file, no error message appears that the file had been modified outside Illustrator. We proceed with the job. Problem - one element that doesn’t exist in ACROBAT, appears in Illustrator. I believe this is a bug, because usually the message is shown.
This problem gave us a big complaint by the customer because that element is forbidden in the Portuguese market.

Generally PDFs can’t be 'opened' in Illustrator, they can be only imported. But if it is a file that was 'saved' in Illustrator as PDF, than you have two separate 'layers' inside of your document: native Ai’s and actual PDF. Therefore you can have a case like this. But you know it for sure :)
I get an error when I try to open 'Our File.ai' (because I don’t have a plugin you use).
Why the warning did not triggered for you, I have no idea, it usually does, indeed. But the AI and PDF (viewed or imported) look different. The box stamp layouts are not the same, text elements are missing, dimensions differ...
