After going through these details, it looks like the file itself open fine / correct but when the thumbnail is used in another file where it is linked, the issue surfaces for you.
Do you see the thumbnail preview of the file correct in Windows folder on disk?
As of now, I think you are seeing an issue similar to this one reported at https://illustrator.uservoice.com/forums/601447-illustrator-desktop-bugs/suggestions/50776133-thumbnails-glitching-when-saved
An error occurred while saving the comment @Ken Rice, @F J - Please note that existing files would not be auto corrected with this fix. But the fix would prevent any future occurrence. So, the best way to check this on 30.2.1 is to correct the files and save it again with version 30.2.1
Let us know if you are still seeing issue after doing this or for a new file created in 30.2.1

1. Tell us about your OS version. Has there been any recent updates to OS?
2. Are you able to launch Illustrator without opening any files?
3. Is the issue happening for each file? Are you able to create new files and save them? -

@JP Flexner - I confirm that we have received the files shared by you. Team is going to look into this.
An error occurred while saving the comment @JP Flexner - this is a good observation. If your pc is currently in the state with preferences showing this issue, please share those preferences with us - same way through email.
You can send us this folder (zipped) - ~/Library/Preferences/Adobe Illustrator 30 Settings/en_US/ which has prefernces and other relevant files.
An error occurred while saving the comment @Jp Flexner - We investigated the files shared. We were able to open the files in latest released version of Illustrator - 30.2.1 and see the content. Are you on latest version? And let us know more about your system details. Do you run into any error on opening the files?

1. Is the issue happening for ai files or pdf files (or both of them)?
2. Try a simple test by creating a new ai file and copy pasting text. Then save the same file as pdf and copy paste again. Does the issue issue happen for both type of files?
3. For the files where issue is happening at your end, were they created in v29? -

An error occurred while saving the comment Thanks for confirmation Camille. In one of your comments made on January 13, 2026, you attached a screenshot showing Ai file when opened, Ai file preview in Finder and preview of a PNG file (created from Ai file) in Finder.
In that screenshot, only PNG file's preview was glitched. Is that a right understanding? Or, do you see Ai file preview in Finder as glitched as well?
Is it possible for you to share any such Ai file with us at sharewithai@adobe.com? Do mention the link of this thread (https://illustrator.uservoice.com/forums/601447/suggestions/50776133) in the subject of email.

To Akshaya Saxena: Yes, the issue is occurring regularly. The file itself is intact, but the thumbnail and the preview in the Finder window appear glitched. Embedding the placed file does not seem to resolve the issue.

Is the incorrect thumbnail issue happening regularly at your end? The fix mentioned for this post is for the original issue of duplication of layers. In your case, the Illustrator file itself is correct but the thumbnail is glitched? Is that correct understanding of the issue?
You also mentioned that when you place this file into another Illustrator document, you run into same issue. Does embedding the file after placing corrects the problem?

Apologies for you running into this issue.
Can you share more details about the nature of these files?
You mentioned that they are embedded. Did you place these files in Illustrator through any particular route - for example dragging from another application to Illustrator? Or, if you take a screenshot and placed it directly in Illustrator?
Do you run into any crash before seeing this issue or does it happen with normal (successful) save and reopening of files?
Do these images have any embedded color profiles?
Try turning off "GPU Performance" checkbox in Performance section of Preferences and relaunch Illustrator. See if that change helps in such files or not.

The team has identified an issue for a small subset of users who are on the Windows operating system and also have their network traffic routed through a proxy.
A temporary solution to this issue is the following:
- Open up PowerShell
- Copy and paste the following commands:

The latest Beta Build 29.6.25 has this bug fixed. Please update and see if it truly works as expected

We tried this at our end with the file shared and could not see the issue.
Are you running into this issue only for the shared file or a new file (starting from scratch) shows similar issues? -

An error occurred while saving the comment In addition to Egor's comment, would it be possible to share a video showing the issues being faced? And a file would help as well.
@Tristan - one of the ways to narrow down Illustrator problem would be to see if the mouse works fine with other applications. Do you see this issue only with Illustrator?
