Rectangle tool draws slightly rotated rectangle
Update 24.2 - When drawing a rectangle (by selecting the rectangle tool, clicking on the artboard, and entering desired dimensions), the resulting rectangle is slightly rotated, skewed or tilted. It's not noticeable until it's viewed in outline mode.
To confirm this, I create a 1" x 1" square with the rectangle tool. Then drag the ruler origin to the top left anchor point. The transform palette confirms this anchor point is at x=0, y=0 (as expected). Then I select the top right anchor point and see that its coordinates are x=1, y=-.0022. The y-value should be 0.
As per the comment, the solution might be to check the Constrain Angle value in Preferences > General (Cmd/Ctrl + K) — it should be 0.

This was driving me nuts. I finally found that if you go to Preferences > General (Command K), make sure the Constrain Angle (second setting from top) is set to 0 degrees. Then hit ok. It should be correct now. I accidentally set mine to 1 and was drawing rectangles at a 1 degree angle. Hope this solves your issue!

I found that I had accidentally hit "command+shift+i" before trying to draw the shape.... when that function was activated, my rectangle tool created rectangles that weren't square. If you hit "command+shift+i" again, and then try and draw a square, it should be fixed!

Hm. For some reason that 1 inch square has a rotation angle specified in the Shape section of the Transform panel, 0.125°... If you reset it to zero, the width and height (measured, in W and H fields) now match rectangle’s dimensions.
Why was this angle set in the first place, I wonder... I see no way to get the rotation inherited on the creation, as you described it.
Can you all please try to test it a bit more to see when and how the angle is forced? -

I am having the same problem... I draw a new rectangle using the rectangle tool, and the rectangle is slightly rotated.
Thanks a lot Adobe, THIS NEEDS TO BE FIXED IMMEDIATELY! This really stuffed me up recently when I saved a load of my clients' logo file types... over 140 logos saved, all with this slight rotatation that i didn't notice until I zoomed in on one of the logos. What a waste of my time!

Seriously Adobe, I am growing so tired of all these bugs! What is going on with your quality control? Do you have work experience kids working on CC apps these days or what?

I have also experienced this issue. When viewed in outline mode, the sides of the rectangle are one pixel off from straight. And if you select the two points at the ends of the offending side and align them with the align tools, they do indeed move into alignment, proving the rectangle tool placed them incorrectly. It's barely noticeable, but if you stretch the rectangle out using the transform tools, the non-straight sides get even more dramatic.
Seems like a failure of the rectangle tool to not perform its most basic function.
