When a crash happens, the Recent Files lists get reset to the state they had on launch
File > Open Recent Files list and a Recent files at home screen don’t get recorded somewhere safe as soon as a document gets opened.
So each time Illustrator crashes (and sadly it’s a ordinary way and the only reason for Illustrator to get relaunched), both lists get lost — Illustrator shows them in the state they had when the program was closed successfully.
Illustrator should update and store these lists as soon as they change and don’t loose them on crash.

Two projects from late last year keep showing up as 'recent', and they show up 10 - 12 times each. What didn't work: I reset the number of recent files in preferences to '0' and restarted Illustrator, then reset that number to 30 again.

I'll look out for steps to reproduce and let you know if I spot a pattern here.
It's not to do with how they're sorted. Its that sometimes files I've previously had open simply don't appear. This always happens after Illustrator has crashed (not infrequently), but it also happens (not infrequently) after shutting down correctly. I've seen the issue multiple times accross multiple installs of multiple recent versions of Illustrator. -

Have the similar issue with Recent Files list on the Welcome page. The list is not updated until restart of the application. E.g. you can create some new files and save them but they do not appear in the recent files welcome page. But they do appear if you restart the application.

When I open Illustrator, I look at the most recent files (especially after Illustrator keeps crashing my 2018 laptop - Power State Failure) and the most recent .ai files aren't there, but the files that I worked on weeks and weeks ago. I don't think Illustrator has ever shown my "most recent," and I have to constantly search for the file in the location on my computer.

"Recent" view doesn't show recent files. (The feature does not work).
It doesn't show the most recently opened file.
When a file is clicked, the WRONG file opens. The thumbnail images and filenames don't match the files that AI opens when I click them.
