Illustrator will automatically convert color codes of shapes and vectors when a document of different color profile is imported.
I've seen this issue where people complain about color codes in Ai changing automatically, this leads to brand color codes slight off for most projects. I've discovered this issue myself and apparently when profile mismatch warning box are not checked (i.e Ask when opening, Ask when pasting) Ai will automatically convert the colors without asking. If you import multiple images then the color will be way off after 10 images. This happens when Ai is in sRGB and the photos are either untagged or AdobeRGB or other than sRGB. If you checked the warning boxes Ai will prompt you what you want to do and by pressing cancel the color codes will remain the same. This should not be a default behavior for color management as this will create a lot of confusion and unpredictable behavior for those who didn't read into color management. I think people will get help from this and please Adobe change it to not convert (preserve numbers as default behavior). This only affect the non global swatch colors. If you assigned a global swatch to the color. It will still remain the same.

As Thar Lynn Oo commented, this happens during drag and drop. The document color profile will be changed and the appearance of the objects too. This cannot be undone.

This is still happening when, as Thar Lynn Oo noticed in this thread, drag and drop is used from Photoshop to Illustrator , the document color profile will be changed to that of the image.
You cannot undo this profile change And the appearance of all the colors will change. -

Adam, is the name of the spot color you created as LAB the same as the name of another spot color located in one of Illustrator's (or your own custom) color books?

No one has addressed when this happens in LAB. I can make a spot color based on LAB, print the file, match it to a dE 1>. Then I cut and paste into a new file, and the SPOT COLOR changes from LAB to BOOK COLOR.

@Rebecca No worries. Always turn on those checkboxes and you should be fine. If you see Ai asking with “Source: Multiple” you will know it is going to do weird stuff with colors. Just press cancel and you should be fine.

I noticed this happens in drag-n-drop import. Doesn't happen in Place (Shift-Cmd-P)! mode.

Yes that should be default behavior or at least ask when mismatch is happening by default.

In short, you want Ai to preserve numbers when profiles mismatch by default, am I right?
